(1)(a) Every person must file a Florida Fuel or Pollutants Tax Application (Form DR-156, incorporated by reference in Rule 12B-5.150, F.A.C.), with the required attachments, with the Department for a pollutant license before producing, selling, importing, or causing perc to be imported into Florida and obtain a pollutant license from the Department.

(b) Each license is required to be renewed annually by filing form DR-156R, Renewal Application for Florida Fuel/Pollutants License (incorporated by reference in Rule 12B-5.150, F.A.C.), and the required attachments with the Department, as provided in the renewal application.

(2) Producing in, importing into, selling in, or causing perc to be imported into Florida for any purpose without first obtaining a license or after such license has been canceled by the Executive Director or the Executive Director’s designee is prohibited.
