(2) The initial license fee is $200.

(3) The biennial renewal fee for an active license is $275.

(4) The biennial renewal fee for an inactive license is $150.

(5) The application fee for inactive status is $200.

(6) The application fee for retired license is $50.

(7) The retired status fee is $50.

(8) Change of status fee for any time other than renewal is $200.

(9) The fee for reactivation of a license is $275.

(10) The deliquency fee is $200.

(11) The initial continuing education provider registration fee is $100.

(12) The biennial continuing education provider registration fee is $100.

(13) The fee for a duplicate wall certificate or license is $25.
