Application, Examination and Initial Licensure Fees

The following fees are established by the Department:

(1) An applicant for licensure by examination shall remit an application fee of $400.00, an examination fee of $400.00, and an initial licensure fee of $400.00 at the time the application is submitted.

(2) An applicant for licensure without examination shall remit an application fee of $400.00 and an initial licensure fee of $400.00 at the time the application is submitted.

(3) An applicant for a residency or an internship shall remit an application fee of $200.00 and a registration fee of $200.00 at the time the application is submitted.

(4) All application fees are non-refundable. Examination fees are refundable upon written request by the applicant if an applicant is found to be ineligible to take the examination. Initial licensure fees are refundable upon written request by the applicant if the applicant is found to be ineligible for licensure.
