Temporary Certificate for Practice in Area of Critical Need.
(1) An applicant for a temporary certificate for practice in an area of critical need shall submit a complete Temporary Certificate for Advanced Practice Registered Nurses to Practice in an Area of Critical Need application, form number DH-MQA 5087, 06/24, hereby incorporated by reference, and may be obtained from https://www.flrules.org/Gateway/reference.asp?No=Ref-16981, demonstrating that he or she meets the qualifications prescribed by the Nurse Practice Act, Chapter 464, F.S. This form is available on the Board’s website: http://floridasnursing.gov.
(2) Within 30 days of accepting employment, the certificateholder shall notify the board at [email protected] of all approved institutions at which he/she is employed.
