(1) For new rates under new prepaid health clinic contracts, the PHC must submit a copy of the new rates, and a full explanation of how the rates were calculated, including utilization assumptions, signed by the preparer.

(2) For new rates under existing prepaid health clinic contracts, the PHC must submit a comparison of the new rates and the existing rates. The PHC must also describe who calculated the rates, and submit a full explanation of how the new rates were calculated, including utilization assumptions and a description of past experience.
