(1) Each financial institution, as defined in s. 655.005, Florida Statutes, must attest, under penalty of perjury, whether the entity is acting in compliance with Section 655.0323(1) and (2), F.S. Such institutions shall attest by completing and submitting Form OFR-U-323, Attestation of Compliance, effective 08-2024, herein incorporated by reference and available at https://www.flrules.org/Gateway/reference.asp?No=Ref-16901 and on the Office’s website, www.flofr.gov, to the Office by July 1 of each year.

(2) Filings required by this rule shall be submitted to the Office by mail or email as follows:
