Florida Armed Forces Reserve Family Readiness Program Application Fund
Application process for the Florida Armed Forces Reserve Family Readiness Program Application Fund (FLAFR FRPAF). All persons applying for assistance from the FLAFR FRPAF shall include all information required by Section 250.5206, F.S., and any other information determined to be needed by the federal Family Center Support Specialist accepting the application for review. Persons applying for assistance shall utilize the FLAFR FRPAF Application for Assistance Forms (dated November 2007), which are incorporated by reference, and available on the Department of Military Affair’s (DMA’s) website at www.dma.state.fl.us. The forms to be completed are the Application for Assistance (Form FRPAF 1-A); Financial Affidavit (Form FRPAF 1-B); Certification of Financial Affidavit (Form FRPAF 1-C); and Processing Form (Form FRPAF 1-D).
