Sec. 17-24 (b) When moving for default and judgment pur- suant to Sections 17-25 through 17-28, a party shall move for default and judgment on forms pre- scribed by the Office of the Chief Court Adminis- trator. (P.B. 1978-1997, Sec. 357.) (Amended June 29, 1998, to take effect Jan. 1, 1999.)
