In the case of a conversion, the national bank shall file with the department: (1) Information desired by the department in order to evaluate the proposed conversion, in the form specified by the department; (2) Applicable fees established by regulation of the department to defray the expenses of its investigation under Code Section 7-1-553; and (3) A certificate of the Secretary of State showing that the proposed name of the resulting bank or trust company has been reserved under Code Section 7-1-131.
