(A) Requirement
A court that has jurisdiction to convict a person of a misdemeanor offense of violence against a family or household member shall provide notice to the defendant pursuant to R.C. 2943.033.
(B) Form
In every case prior to accepting a guilty plea or plea of no contest to an indictment, information, or complaint that charges a person with a misdemeanor offense of violence against a family or household member, a court shall use a form that is substantially similar to a “Notice Concerning Possession or Purchase of Firearms” or Form 5 (formerly 10.04- A)” unless the court provides oral notice to the defendant.
