The 2015 amendment, effective July 1, 2015, in subsection (c), deleted ", in triplicate," following "department" in the first sentence, substituted "a copy" for "one copy" in the third sentence, in the form, deleted "known as" following "Annotated," and substituted "such proposed" for "said proposed", and deleted "the" preceding "statement" in the last sentence.
